Endotoxin Testing Is Not Sterility Testing: What Each Test Can and Cannot Tell You
A clear explanation of bacterial endotoxin testing, sterility testing, why the two are separate, and what a passing result does—and does not—establish.
Endotoxin testing and sterility testing are often mentioned together in discussions of injectable products, but they are not the same test and they do not answer the same question.
What endotoxin testing looks for
Bacterial endotoxins are lipopolysaccharide components associated with the outer membrane of Gram-negative bacteria. They can remain biologically active even after the bacteria that produced them are no longer viable.
USP <85> Bacterial Endotoxins Test and related methods are designed to detect or quantify bacterial endotoxins. FDA’s 2026 guidance discusses gel-clot, photometric, and kinetic approaches and emphasizes appropriate testing of components and finished products where endotoxin control is required.
What sterility testing looks for
Sterility testing addresses a different problem: the presence of viable contaminating microorganisms. USP <71> is the compendial sterility-test chapter used for articles that are required to be sterile.
FDA and USP materials treat sterility and endotoxin as separate quality attributes because a sample can perform differently on the two tests.
Why “low endotoxin” does not mean “sterile”
A low endotoxin result does not establish that no viable bacteria, fungi, or other microorganisms are present. Endotoxin testing is focused primarily on endotoxin from Gram-negative bacteria, not on all possible microbial contamination.
The reverse is also important: a sterility test does not automatically establish an acceptable endotoxin level. Endotoxin can persist after bacterial cells have died or lysed.
FDA specifications make the distinction clear
FDA’s Q6A guidance discusses parenteral-product specifications and lists sterility and endotoxin or pyrogen testing as separate items. It also separately addresses particulate matter. The practical implication is that a single passing quality test should not be used as a stand-in for the other attributes.
Testing is only one part of sterility assurance
For products that are legitimately manufactured as sterile products, sterility assurance depends heavily on validated manufacturing and aseptic-processing controls. FDA’s aseptic-processing guidance emphasizes validated sterilization, aseptic filling, environmental control, and process simulations. End-product testing alone cannot compensate for an uncontrolled process.
How to read a laboratory report
When reviewing an endotoxin or sterility report, look for:
- The exact lot or sample identifier.
- The method used.
- The laboratory and test date.
- The specification or acceptance criterion.
- The reported result and units.
- Any dilution, interference, or method-suitability information relevant to interpretation.
Research-use context matters
Nothing about a passing endotoxin or sterility result converts an unapproved research material into an FDA-approved drug or establishes that a research product is appropriate for human administration. Analytical results describe the attributes actually tested; they do not create a regulatory approval or a clinical-use recommendation.
Bottom line
Endotoxin testing asks whether bacterial endotoxin is present at the level evaluated by the method. Sterility testing asks whether viable microbial contamination is detected under the sterility method. Both can be important in legitimate parenteral manufacturing, but one cannot be substituted casually for the other.
